Accepted Insurance Carriers:
Our office gladly accommodates families covered by orthodontic insurance. We ask only that you read and understand your policy, carefully evaluating any limitations of the benefits. The fees we charge for services rendered to those who are insured are our usual and customary fees charged to all patients for similar services. Your policy may base its allowances on a fixed fee schedule that may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage available. Also, some companies pay claims promptly and others delay payments many months.

We ask that you view your insurance realistically as a device that reimburses YOU for dental expenses. It is easiest for us to keep track of your account if your insurance company pays you. Therefore, we would prefer that you do not assign payment of benefits to our office. Our obligation is to complete all forms pertaining to your claim and send them promptly to your company to help you obtain the
reimbursement to which you are entitled. If for any reason your insurance is discontinued during treatment, the unpaid balance becomes the liability of the responsible party.

Payment Options:
The total cost of treatment includes all costs associated with the initial exam, diagnostic records, consultation, appliances -braces, headgear, retainers, etc. -plus all appointments needed for adjustment of the appliances until treatment is completed.
Should an appliance become lost or broken due to patient neglect and carelessness, a separate replacement fee will be charged. The fee does not include any fillings, x-rays, surgery or special records performed outside of our office during the course of orthodontic treatment.

Our customary procedure in handling our patients' accounts is the same for patients with or without insurance coverage. The fee is set and an initial payment is due from you at the start of treatment.  The payment plan for the remaining fee represents a convenient means to budget payments over a period of time. The payment schedule does not relate to the number of office visits. Please remember that the monthly payment is due regardless of appointment scheduling. It is not our policy to send out monthly statements unless an account becomes overdue. The fee must be paid in full before treatment is completed. Payment by cash and check are accepted, along with MasterCard and Visa.  We reserve the right to discontinue treatment for lack of cooperation, failure to keep a high standard of oral hygiene, or lack of fulfillment of financial arrangements.

Appointment Scheduling Details:
Initial Exam Appointment -This first examination allows us to get acquainted. At this time, we will be able to give you a professional assessment of the orthodontic problem, treatment options, length of treatment, and approximate fee for total orthodontic treatment.  If you choose to begin treatment, the cost of this initial visit will be incorporated into the total fee.

Diagnostic Records Appointment -If the initial exam reveals that you, or your child, can benefit from orthodontic treatment, then diagnostic records are necessary. During this visit, we take models of your teeth, photographs, cephalometric (head) radiographs, a panorex x-ray, and a complete dental history. These detailed records enable us to determine the best treatment plan for you.

Consultation Appointment -After studying the diagnostic records, a Consultation Appointment is arranged so that we can fully explain the nature and extent of the patient's orthodontic problems. During this appointment we will present a total treatment fee and the various payment plans available to you at no interest.  We will also discuss any insurance coverage you might have and the portion of the fee your policy might pay.

Office Appointments -We schedule the attachment and removal of braces, and bracket repairs during morning and early afternoon hours due to the length of time required for these involved processes. After the braces are placed on the patient's teeth, appointments will be scheduled every six to eight weeks for adjustments. Since many of our patients attend school, in all fairness, we cannot give preferential scheduling for late afternoon appointments. Most patients request after school or work appointments. For this reason, there may be times when you will not be seen right on schedule. If this is a problem, we suggest morning or early afternoon appointments.
If you are experiencing a problem with loose brackets or bands, we ask that you call as soon as possible during regular office hours. Please don't wait to inform us of a problem when you arrive for your regularly scheduled check. It may be necessary to schedule a second appointment for the repair, requiring an extra trip to the office.
If it is impossible to keep your scheduled appointment, please notify us 24 hours in advance so that your reserved time may be offered to another patient. Being on time for all appointments is essential so that the full time allotted for treatment may be utilized. Patients who miss or postpone their appointments extend their treatment time and compromise the goals we are helping them achieve.

Emergencies -Although Dr. Bogenschutz does not normally see patients on Fridays, weekends, or holidays, he is available to provide emergency care. If there is considerable discomfort or pain from the braces or other appliances, please call our office. When the office is closed, you many call and leave a recorded message; we will return your call as soon as possible.
